You can wear just one. Each earpiece has full controls. Unless you have the volume really loud, you can hear what is going on around you. I run with both earpieces.

Each side has it's own Bluetooth receiver. Because you can wear one earbud with full functionality while the other is in the case and inactive, both sides have to have all of the same hardware. I am late to the game. I ordered mine on the 18th, and I was to wait for a month for it. Then Amazon sent me an email to update on shipping date. It said I will be expecting delivery on June 3rd. I was all happy, until they cancelled that date. Now it just says will update as soon as they have a new date. :(
